If you received a letter in the mail with a PIN, please read the directions below and click the red form button to complete. 

DIRECTIONS:

  1. Locate your PIN (see image to the right).
  2. Enter your PIN and date of birth. Both must match in our database to complete this process. If you receive an error, we may have an incorrect date of birth on file.

NOTE: The online form does not support all mobile devices.
