Directions

If you received a letter in the mail with a provided PIN, please complete the form below: 

  1. Using the letter you received, identify the type of letter and locate your PIN.
  2. Enter your PIN and date of birth. Both must match in our database to complete this process. If you receive an error, we may have an incorrect date of birth on file for you.

NOTE:  If you complete the online 3C form with your PIN, do not return the mail-in form you received by mail.
